To register with Baby Nurse and Doulas etc.:

Requirements:

Main certification - must have one from either a.) or b.) :

a.) Postpartum Doula Certificate through DONA
                                      or
      Newborn Care Specialist through NCSA

b.) RN degree or Licensed Practical Nurse

 

Must have:

-Breastfeeding Educator certification

-Postpartum Liability Insurance policy.

 

Please fill out our application and return by email, fax or in person (by appointment only) along with the copy of the following:

-Drivers License or State ID Card
-Social Security Card
-Work Permit or Permanent Resident card
-CPR card
-TB test results, if applicable
-Copies of your certifications
-Copies of any reference letters
-Current Resume
